You must provide a Video of yourself welding in the process you are qualified in (i.e., SMAW, GTAW, FCAW, etc.). This form is good for only ONE PROCESS. If you wish to do more than one Assessment, you must purchase and submit this form again for each process you wish to be assessed in.

- You welding in the process & metal you qualified in.
- A shot of the finished weld (welds must be clean and visible)
